Sports Physicals
Sports Physicals are scheduled for two dates in September. If your child is in 6 – 12th grade, and wishes to participate in sports during the school year, they are required to have a Sports Physical and cleared for participation. The physical must be on the NJ Sports Physical Form, which can be found in the links below. No other form can be accepted. Please complete the Parent Questionnaire and return all forms to school prior to the scheduled dates.
Physicals are performed at Chapel Hill Academy in the Health Office by our School Physician or Physicians Assistant. There is no charge for the Physical. If you choose to have your own private Physician perform the sports physical, please bring the NJ Sports Physical form with you, as this is the only form that can be accepted, in order to be compliant with NJ School Regulations.
